How do I report a side effect to the MHRA?

The Yellow Card scheme is the UK's official way to report suspected side effects or problems with medications. It is run by the Medicines and Healthcare products Regulatory Agency (MHRA).

What you can report

  • Side effects, even mild ones
  • Suspected adverse reactions
  • Faulty or defective medicines (broken pens, dose delivery problems)
  • Suspected counterfeit medicines
  • Quality concerns (packaging issues, wrong-looking medication)

How to report

Anyone can report — you do not need to be a healthcare professional.

  • Online: yellowcard.mhra.gov.uk
  • By app: Search "Yellow Card MHRA" in the App Store or Google Play
  • By post: Forms are available from the MHRA website

What to include

  • The medication name (Mounjaro / tirzepatide, Wegovy / semaglutide, Saxenda / liraglutide)
  • The batch number if you have it (printed on the pen and box)
  • What happened, when, and how long it lasted
  • Any other medications you were taking at the time

Will it affect my treatment?

No. Yellow Card reports are confidential and do not affect your relationship with us or your treatment. The MHRA uses reports to spot patterns and keep medicines safe for everyone.

Should I tell PharmacyOnline as well?

Yes — please contact our team. We may submit our own Yellow Card report for serious or unexpected events, and we want to support you with the side effect or issue you've experienced.
